Hemi resurfacing of the humeral head for an osteochondral lesion affecting the entire head surface.

چکیده
An osteochondral lesion of the entire articular surface of the humeral head has not previously been reported. We report the case of a 73-year-old woman with a huge osteochrondal lesion which affected virtually the whole articular surface of the humeral head, caused by an unusal mechanism of injury. Clinical examination revealed a stiff, weak, painful shoulder. Radiographs were unremarkable and an ultrasound scan suggested a supraspinatus tear with retraction. At arthroscopy the lesion was identified but the lesion was to large for arthroscopic removal. A humeral head hemi-resurfacing as a treatment for this lesion produced excellent results, the patient returned to her pre-injury level of function with no pain.

Osteoarticular allograft reconstruction for hill-sachs lesion in an adolescent.
Glenoid and humeral head bone loss is the most common cause of failure after surgical shoulder instability stabilization. Large Hill-Sachs lesions involving >30% of the articular surface of the humeral head typically occur after neglected or locked shoulder dislocations. Such large lesions may require osteochondral allograft reconstruction or prosthetic replacement to regain shoulder stability ...
متن کاملAvascular Necrosis of the Humeral Head: Hemi-Cap, Cap, or Stemmed Solution?
Avascular necrosis of the humeral head, as in other joints, can have a spectrum of severity. The degree of humeral head involvement should dictate the degree of prosthetic replacement required to restore congruity and function. In some very early phases of disease, observation or core decompression and supportive bone grafting may be considered. This article will focus on management of later pr...
متن کاملComparison of arthroscopic osteochondral substitute grafting and remplissage for engaging Hill-Sachs lesions.
The optimum management for recurrent glenohumeral instability with significant humeral head defects remains controversial. The purpose of this study was to compare outcomes and recurrence rates between patients who underwent osteochondral substitute grafting and patients who underwent remplissage to treat recurrent shoulder instability from an engaging Hill-Sachs defect with a Bankart lesion. T...
متن کاملFresh Osteochondral Allograft to the Humeral Head for Treatment of an Engaging Reverse Hill-Sachs Lesion
Posterior dislocation of the shoulder is rare, occurring in approximately 2% to 4% of all shoulder dislocations. Through a mechanism similar to that first characterized by Hill and Sachs, these dislocations have the potential to result in impaction fractures of the anterior aspect of the humeral head. These so-called reverse Hill-Sachs or McLaughlin lesions are estimated to occur in 86% of acut...
متن کاملHumeral Head Reconstruction With Osteochondral Allograft Transplantation.
PURPOSE To synthesize, in a systematic review, the available clinical evidence of osteochondral allograft transplants for large osteochondral defects of the humeral head. METHODS The Medline, Embase, and Cochrane databases were searched for studies reporting clinical or radiographic outcomes of osteochondral allograft transplantation for humeral head defects. Descriptive statistics were provi...
